29 CFR  1910.147(c)(7)(i):The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:  a) Mennel Bakery and Food Services LLC. on or about July 16, 2022, at the address of 4136 Martel Road Caledonia OH; The employer did not to ensure that employees who are tasked with cleaning activities on the Accel Pak Bagger/Aero-Seal Bag Closer on line 1 were adequately trained on the purpose and elements of the safe application and use relating to the isolation and control of hazardous energy sources. Employees tasked with cleaning activities at or near the in feed belt (nip-point) on the aforementioned machine, were not specifically trained to lockout the machine relating to the specific task.

Employees performing infeed belt cleaning in an energized state were exposed to caught-in hazards.

29 CFR  1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a)	Mennel Bakery and Food Services LLC. on or about September 22, 2022, at the address of 4136 Martel Road Caledonia OH; The employer did not ensure that the point of operation of the Accel Pak Bagger/Aero-Seal Bag Closer on line 10 packing machine was adequately guarded. Employees handling bags of bakery mix near the unguarded point of operation were exposed to caught-in and pinch point hazards.
