The employer did not ensure that for greater heights, outriggers, counterweights, or comparable means that stabilize the mobile ladder stands and platforms and prevent overturning are used:  Shop:  On, or about, October 3, 2022, a Werner Model SRS-72 rolling scaffold was used with the upper work platform located more than four times the base dimension of the platform wheels.    The outriggers were not installed in the proper position to prevent the scaffold from tipping.
