29 CFR  1904.39(a)(2):The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.   A.) On or about October 14, 2022: Juniper Elbow Co. Inc. 72-15 Metropolitan Avenue, Middle Village, NY, 11379.  Employer failed to report an inpatient hospitalization within 24 hours of the incident.  OSHA was notified of the incident by online reporting on October 17, 2022.  ABATEMENT CERTIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
