29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  A) Cake Decorating Room: The Chanmag mixer's rotating shafts and blades were not guarded.  Employees were exposed to the rotating part hazards.  B) Production area:  The large Hobart mixer's rotating shafts and blades were not guarded.  Employees were exposed to the rotating part hazards.  C) Production area: The small Hobart mixer's rotating shafts and blades were not guarded.  Employees were exposed to the rotating part hazards.  The Guam Bakery was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1414433, citation number 1, item number 1 and was affirmed as a final order on 10/17/2019, with respect to a workplace located at 140 Kayen Chando, Dededo, Guam, 96929.

Employees were exposed to fire hazards such as smoke inhalation and burns.  B) In the loading dock area: The ABC fire extinguisher was blocked by storage and large pallets.  Employees were exposed to struck-by hazards while removing the fire extinguisher from its location and fire hazards such as smoke inhalation and burns.

Access was restricted in the event of an emergency.  B) Right of the loading dock door: Electrical panel was blocked by bags of old bread.  Access was restricted in the event of an emergency.  C) Behind the Svebadahlen steam oven: Electrical panel 6 was blocked by baking racks and a trach can. Access was restricted in the event of an emergency.
