29 CFR 1910.29(f)(4): Openings in stair rails. The employer did not ensure that no opening in a stair rail system exceeds 19 inches (48 cm) at its least dimension.  On or about October 24, 2022, and times prior thereto, the employer did not installed standard rails such as mid-rails on stairs utilized to access upper landings to conduct work in and around tanks exposing employees to fall hazards.  a) At the northeast side of the storage tanks, employees access tanks using a stairway missing mid-rails,  b) At the southeast side of the storage tanks, employees access tanks using a stairway missing mid-rails,  c) In the waste treatment department, employees access tanks using a stairway missing mid-rails.

29 CFR  1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.  On or about October 24, 2022, and times prior thereto, the employer did not ensure fire extinguishers were identified with a fire extinguisher sign and ready accessible for employees exposing to fire and smoke hazards.  a) At the northeast exit door, fire extinguisher sign missing above fire extinguisher,  b) At the southwest exit door, fire extinguisher sign missing above fire extinguisher.

29 CFR  1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure.  On or about October 24, 2022, and times prior thereto, an extension cord, wrapped tied around a metal storage, was used as permanent wiring to energize an Optima digital weight scale, exposing employees to electrical shock hazards.
