29 CFR  1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.  On or about November 17, 2023, and at times prior thereto, west side of the warehouse, the employer did not ensure employees had clear access to the fire extinguisher exposing employees to fire and smoke hazards.

29 CFR  1910.303(b)(1)(iv):Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation:  On or about November 17, 2023, and at times prior thereto, north side of the warehouse, the employees were exposed to electrical injuries due to an electrical junction box missing the cover plate.

On or about November 17, 2023, and at times prior thereto, west side of the warehouse, an electrical panel box that energizes lights and machines was left open exposing employees to electrical shock hazards.
