29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts:  a) Plant; on or about  October 28, 2022 and at times prior to, the employer exposed machine operators to amputation hazards, in that the horizontal band sander Asset ID 1082 (combined with wire brush wheel) was not guarded to protect workers from ingoing nip points and rotating parts.   b) Plant; on or about  October 28, 2022 and at times prior to, the employer exposed machine operators to amputation hazards, in that the  horizontal band sander Asset ID 1140 was not guarded to protect workers from ingoing nip points and rotating parts.   c) Tube Fabrication Area; on or about  October 28, 2022 and at times prior to, the employer exposed machine operators to amputation hazards, in that the tube forming machine 1 was not guarded on the sides to protect workers from ingoing nip points and rotating parts.  d) Tube Fabrication Area; on or about  October 28, 2022 and at times prior to, the employer exposed machine operators to amputation hazards, in that the tube forming machine 2 was not guarded on the sides to protect workers from ingoing nip points and rotating parts.  e) Tube Fabrication Area; on or about October 28, 2022 and at times prior to, the employer exposed machine operators to amputation hazards, in that the tube forming machine 3 was not guarded on the sides to protect workers from ingoing nip points and rotating parts.  Flexible Metal, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1536806, citation number 1, item number 3, and was affirmed as a final order on January 14, 2022, with respect to a workplace located at 2467 Mountain Industrial Blvd., Tucker, GA 30084.

29 CFR  1910.305(g)(2)(iii):Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  a) Tube Bending Area, Scotchman Circular Saw Asset ID 1087; on or about  October 28, 2022, and at times prior to, the employer exposed operators to electrical hazards, in that the damaged  strain relief device in the  power cable for the emergency push button was not repaired to prevent electric shock or the deactivation of the emergency push button.  Flexible Metal, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1536806, citation number 1, item number 4, and was affirmed as a final order on January 14, 2022, with respect to a workplace located at 2467 Mountain Industrial Blvd., Tucker, GA 30084.
