Location - 21 Ellis St. Framingham, MA;  On or about 11/03/2022, a powered industrial truck operator raised a load with an employee and then left the cab with load/employee suspended. The suspended load was greater than 22 feet above lower level.  The Hulk Construction Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(a)(6), which was contained in OSHA inspection number 1583317, citation number 1, item number 1b and was affirmed as a final order on 6/24/2022, with respect to a workplace located at 203 Marrett Road, Lexington, MA 02420.
