Location -  33 John Street, Newton, MA.  On or about 8/28/2023, seven employees were not protected from falling while using a ladder that did not extend at least three feet above the upper landing surface.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  Location - 33 John Street, Newton, MA.  On or about 8/28/2023, seven employees were exposed to fall hazards greater than seven feet above lower level from a residential first floor deck were fall protection was not used.  Hulk Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1583317, citation number 1, item number 3, and was affirmed as a final order on 6/24/2022, with respect to a workplace located at 203 Marrett Street, Lexington, MA.  Hulk Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1632426, citation number 2, item number 4, and was affirmed as a final order on 4/10/2023, with respect to a workplace located at 21 Elliss Street, Framingham, MA.
