29 CFR  1910.23(c)(4):(4) Ladders were not used only on stable and level surfaces and  they were not secured or stabilized to prevent accidental displacement:  1619 East St., Pittsfield, MA: On or about November 10, 2022, an employee was working from a portable fiberglass extension ladder that was not secured to prevent the ladder from collapsing upon itself.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19 (d) (1).
