29 CFR  1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:   On or about November 09, 2022, employees were exposed to struck by hazard, while performing roofing work on a residential home located at 229 Mooreland Commons Chagrin Falls, Ohio 44022. The employees were using air powered nailers and the employer did not ensure they were wearing any form of eye protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about November 9, 2022, employees were exposed to  fall hazards of 12 feet while performing roofing activities on a residential home located at 229 Mooreland Commons Chagrin Falls, Ohio 44022. The employees were using an extension ladder to access the roof and the employer did not ensure that it extended at least 3 feet above the landing area.
