29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:   On or about December 20, 2023, an employee was exposed to a struck by hazards while working approximately 24 feet below an active roofing crew at a residential home located at 338 Moorland Hills Road Chagrin Falls, OH 44022, and was not provided a protective helmet by the employer.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about December 20, 2023, the employer did not provide a training program for employees exposed to a fall of 24 feet above the ground while they were installing shingles on the roof of a residential home located at 338 Mooreland Hills Road Chagrin, Ohio 44022, to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation:   On or about December 20, 2023, employees were exposed to struck by hazard, while performing roofing work on a residential home located at 338 Moorland Hills Road Chagrin Falls, OH 44022. The employees were using pneumatic nail guns, and the employer did not ensure they were wearing any form of eye protection.  EW Roofing, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1633618 citation number 1, item number 1 and was affirmed as a final order on 3/13/2023, with respect to a workplace located at 229 Mooreland Commons Chagrin Falls, Ohio 44022.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   The employer does not require any form of fall protection during roofing work. This violation was most recently observed on December 20, 2023, at a jobsite located at 338 Moorland Hills Road Chagrin Falls, OH 44022, where employees were not protected from fall hazards of 24 feet to the ground. To abate this violation, the employer must ensure that its employees performing roofing work are protected from falls, and that no employee is allowed onto a roof more than 6 feet high without adequate fall protection.
