29 CFR  1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:   EXTRACTION LABS:  On or about November 10, 2022, the employer did not select proper personal protective equipment to protect employees from exposure to methylene chloride while preparing soil and water samples for analysis of semi-volatile organic compounds in that:  a) Employees wore nitrile exam gloves to protect their hands while adding methylene chloride to reaction vessels and cleaning glassware. Nitrile gloves are not protective against methylene chloride. b) Employees wore safety glasses while dispensing methylene chloride, adding it to reaction vessels, and shaking the contents of the vessels. Safety glasses do not provide adequate eye protection for these tasks.

29 CFR  1910.1052(h)(1): Where needed to prevent methylene chloride induced skin or eye irritation, the employer did not provide clean protective clothing and equipment resistant to methylene chloride, at no cost to the employee, and/or did not ensure that each affected employee used it:   On or about November 10, 2022, the employer did not ensure methylene chloride resistant personal protective equipment was provided to exposed workers performing tasks such as (but not limited to) water extraction and soil extraction in that:  a) Employees wore nitrile exam gloves to protect their hands while adding methylene chloride to reaction vessels and cleaning glassware.
