29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:    At a work site located at 11401 Congress Ave, Kansas City, MO. Employees were exposed to serious injuries, in that the unprotected sides and edges of the walking working surface around open holes were not provided with adequate covers or any other means of fall protection. The employees were exposed to fall hazards of over 4 feet to the concrete surfaces below.
