29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a) On or about August 31, 2022, Expert Exteriors LLC, as controlling employer did not ensure its employee and the sub-contractor employees from GRR Roofing, Inc. were protected from struck-by hazards from debris being thrown down from overhead from a three-story hotel roof. Employees were not wearing any protective headgear (i.e. - hardhats, bump caps, etc.).

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a) As the controlling employer, on or about August 31, 2022, and at times prior,  the employees of a sub-contractor, GRR Roofing, Inc. were exposed to fall hazards while conducting roofing working on a 3 story building with no means of fall protection. Employees were exposed to fall hazards.

29 CFR  1926.1053(b)(6): Ladders were used on surfaces which were not stable and level:  (a) On or about August 31, 2022, Expert Exteriors LLC, as the controlling employer, did not ensure that sub-contractor employees from GRR Roofing, Inc. were protected from fall hazards associated with unstable ladder placement. The feet of an extension ladder used to reach the roof of a three-story hotel were placed in a decorative rock bed on the side of the building with no securement system in place.
