29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   (a) On or about June 29, 2023, employees conducting roofing work on an existing residential home located at 1236 Garfield Ln Hartford, WI 53027 with an approximately 8:12 pitch, were not protected from falling to the lower level. This exposed employees to a fall hazard of up to approximately 19 feet on the front side of the house.

29 CFR  1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  (a) On or about June 29, 2023, at 1236 Garfield Ln Hartford, WI 53027, frequent and regular job site inspections were not conducted to identify and correct safety hazards, including but not limited to fall hazards.
