29 CFR  1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  On or about December 20, 2022, on site of a residential structure under construction located at 9000 Terzi Way in Port Saint Lucie, Florida, employees exposed to fall hazards from a roof and ladder prior to the employer conducting an inspection of the worksite to ensure employees were utilizing all their required personal protective equipment and had a safe manner to raise tools and/or equipment to the roof level.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about December 20, 2022, on the north side roof of a structure under construction located at 9000 Terzi Way in Port Saint Lucie, Florida, employees performing plywood sheathing installation work on the roof of a residential structure were exposed to a fall hazard of approximately 12 feet, 5 inches while not being protected from falling to the lower level.

29 CFR  1926.1053(b)(22):Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  On or about December 20, 2022, at the north side roof edge of a residential structure under construction located at 9000 Terzi Way in Port Saint Lucie, Florida, an employee was exposed to fall hazard of approximately 12 feet, 5 inches when climbing an extension ladder while carrying a rope in hand which could result in him losing his balance and fall.
