29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids, or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about December 27, 2022, and at times prior thereto, employees were using a grinder on a metal canopy tubing without eye protection, exposing the employees to the hazard of being struck-by flying projectiles or debris.

29 CFR  1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:  On or about December 27, 2022, and at times prior thereto, employees were standing on the rails to gain height on a Genie aerial lift while working on a multifamily apartment complex under construction, exposing employees to a fall hazard.
