29 CFR  1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  a.  On or about December 29, 2022, at the above addressed jobsite, employees wearing personal fall arrest systems while performing residential roofing operations were not attached to the lanyard and rope by the D-ring in the center of the employees' backs near should level, thereby exposing the employees to fall hazards.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about December 29, 2022, at the above addressed jobsite, an employee performing residential roofing operations, nine (9) feet above a lower level, was not protected from falls, thereby exposing the employee to fall hazards.  Almanza Roofing Contractors LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1487168, citation number 1, item number 1, and was affirmed as a final order on December 17, 2020, with respect to a workplace located at 8000 Stonegate Dr. in Tinley Park, Illinois.
