29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) On or about May 10, 2023, at the above addressed jobsite, an employee using a pneumatic nail gun was not wearing a safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   a) On or about May 10, 2023, at the above addressed jobsite, an employee performing commercial roofing operations, ten (10) feet six (6) inches above a lower level, was not protected from falls, thereby exposing the employee to fall hazards.  Almanza Roofing Contractors LLC was previously cited for a violation of an equivalent occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1641119, citation number 2, item number 1, and was affirmed as a final order on February 7, 2023, with respect to a workplace located at 2274 Fieldstone Dr. in Frankfort, Illinois.  Almanza Roofing Contractors LLC was previously cited for a violation of an equivalent occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1487168, citation number 1, item number 1, and was affirmed as a final order on December 17, 2020, with respect to a workplace located at 8000 Stonegate Dr. in Tinley Park, Illinois.
