29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On January 6, 2023, L5 Enterprises LLC was constructing a multi residence building located at 19303 Folsom Drive, Warrenton, Missouri.  A pneumatic nail gun was being used to install plywood roof decking on the Northwest side of the building.  The employee was not wearing eye protection, exposing the worker to a struck-by hazards.

Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems:  On January 6, 2023, L5 Enterprises LLC was constructing a multi residence building located at 19303 Folsom Drive, Warrenton, Missouri.  Employee(s) were installing trusses and roof decking to the building.   The pitch of the roof was 5: 12 with a height of approximately 9 ft. at the eave to 20 ft. at the ridge.  The employees did not utilize approved guardrail systems, a safety net system, or a personal fall arrest system.

On the Northwest side of the building a yellow Falcon Heavy Duty self-supporting ladder was being used in the closed position leaning against the building wall.  A employee who was standing on the fifth rung of the ladder from the ground.
