29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   On or about January 4, 2023, and at times prior thereto, I-2/I-69 Interchange Project Direct Connection, the employer failed to instruct employees in methods to recognize and avoid potential struck-by hazards from improperly stacked steel diaphragms and exposed employees to struck by hazards.

29 CFR  1926.250(a)(1):Materials stored in tiers were not stacked, racked, blocked, interlocked, or otherwise secured to prevent sliding, falling or collapse:  On or about January 4, 2023, and at times prior thereto, I-2/I-69 Interchange Project Direct Connection, employer failed to ensure steel diaphragms stored in tiers were stacked and remained interlocked prior to employees conducting rigging operations, as a result, the diaphragms fell and struck an employee, sustaining a serious injury.
