29 CFR  1915.36(a)(2): Frequent tests were not made by a competent person to ascertain that the concentration of flammable vapors was below 10 percent of the lower explosive limit;  USS Kennedy at Newport News Shipbuilding: On 1/18/23, and times prior thereto, employees were exposed uncontrolled concentrations of flammable vapors from  formula 150 Green epoxy and formula 124 soft white enamel, due to the employer failing to test the effectiveness of the ventilation in enclosed spaces.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1915.81(b)(1)(iv):  The employer did not place each hose and cord above the walkway in a location that would prevent injury to employees and damage to the hoses and cords;  USS Kennedy at Newport News Shipbuilding, 7th deck weapon's elevator: On 1/18/23, and times prior thereto, employees were exposed to tripping hazards and a fall of 44", due to ventilation tubes, welding cables, staging, stored materials, and ship structures impeding the walkway passage on and around the threshold between the weapon's elevator and magazine.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1915.157(a):The employer did not ensure that employees used appropriate hand protection or protective clothing when exposed to hazards such as skin absorption of harmful substances, severe cuts, lacerations, abrasions, punctures, chemical burns, thermal burns, harmful temperature extremes, and sharp objects;  USS Kennedy at Newport News Shipbuilding, 5-138-0-M: On or around 1/17/23, employees were exposed to health hazards from not being provided gloves while working with 124 soft white enamel.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
