29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) 188-214 Kinderkamack Road / roof: Three employees working below upper roof had exposure to a struck-by-hazard from staged roofing shingles 12ft above, on or about 1/18/2023.   b) 188-214 Kinderkamack Road / roof: Two employees working without hardhats within 2ft of energized electrical lines, on or about 1/18/2023.      Guicho's Home Improvement, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1470248, citation number 1 item number 1 and was affirmed as a final order on 9/07/2021, with respect to a workplace located at 6 Churchill Court, Paramus, NJ 07652.

29 CFR  1926.102(a)(2): The employer did not ensure that each affected employee used eye protection that provided side protection when there is a hazard from flying objects.   a) 188-214 Kinderkamack Road / roof:  Two employees were observed using pneumatic nail guns to install roofing shingles without eye protection on a 20ft roof of a two story residential house, on or about 1/18/2023.   Guicho's Home Improvement, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(b), which was contained in OSHA inspection number 1470248, citation number 1 item number 2 and was affirmed as a final order on 9/07/2021, with respect to a workplace located at 6 Churchill Court, Paramus, NJ 07652.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501   a) 188-214 Kinderkamack Road / roof:  Two employees replacing roofing shingles were exposed to a fall hazard of approximately 25 feet while on a roof with an 8 on 12 pitch without utilizing a fall protection system in place, on or about 1/18/2023.  b) 188-214 Kinderkamack Road / roof: Two employees replacing roofing shingles were exposed to a fall hazard of approximately 12 feet while on a roof with an 6 on 12 pitch without utilizing a fall protection system in place, on or about 1/18/2023.      Guicho's Home Improvement, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1470248, citation number 1 item number 5 and was affirmed as a final order on 9/07/2021, with respect to a workplace located at 6 Churchill Court, Paramus, NJ 07652.  Guicho's Home Improvement, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1529348, citation number 1 item number 1 and was affirmed as a final order on 01/04/2023, with respect to a workplace located at 560 Churchill Court, Paramus, NJ 07652.
