29 CFR  1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance paragraph (b) or (c) of 29 CFR 1926.652.   a) Jobsite: 846 N. Broad St. Philadelphia, PA 19130, Rear of building - On or about January 18, 2023, the employer did not ensure that an employee replacing a curb trap inside a trench measuring approximately 7 feet and 3 inches in depth, 9 feet and 3 inches in length and 4 feet and 2 inches in width was protected from cave-ins by the use of an adequate protective system.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  a) Jobsite: 846 N. Broad ST. Philadelphia, PA 19130, Rear of building - On or about January 18, 2023, the employer did not ensure that an employee replacing a curb trap inside a trench measuring approximately 7 feet and 3 inches in depth, 9 feet and 3 inches in length and 4 feet and 2 inches in width was provided with a safe means of access by extending the ladder inside the trench at least 3 feet above the upper landing surface of the trench.
