Broad St. Philadelphia, PA 19130, Rear of building - On or about January 18, 2023, the employer did not ensure that an employee replacing a curb trap inside a trench measuring approximately 7 feet and 3 inches in depth, 9 feet and 3 inches in length and 4 feet and 2 inches in width was protected from cave-ins by the use of an adequate protective system.

Broad ST. Philadelphia, PA 19130, Rear of building - On or about January 18, 2023, the employer did not ensure that an employee replacing a curb trap inside a trench measuring approximately 7 feet and 3 inches in depth, 9 feet and 3 inches in length and 4 feet and 2 inches in width was provided with a safe means of access by extending the ladder inside the trench at least 3 feet above the upper landing surface of the trench.
