29 CFR  1910.27(a): The employer did not ensure that scaffolds used in general industry meet the requirements in 29 CFR part 1926, subpart L (Scaffolds).  Location:  Warehouse  On or about January 25, 2023, the mobile scaffold swing-gate was fastened in the open position while employees operated the lift.  The mobile scaffold did not have the top-rail, mid-rail, or toe board in use on all sides exposing employees to fall and struck by hazards.

29 CFR 1910.140(c)(22): The employer did not ensure that personal fall protection systems are worn with the attachment point of the body harness located in the center of the employee's back near shoulder level. The attachment point may be located in the pre-sternal position if the free fall distance is limited to 2 feet (0.6 m) or less.   Location: Warehouse  On or about January 25, 2023, an employee was not wearing a safety harness properly and was exposed to fall hazards.  The employee was wearing a personal fall protection harness with the adjustment straps loose and the attachment point of the harness was closer to the employees waist than the center of the back.
