29 CFR  1910.178(l)(6):The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation:  Jobsite: Warehouse   On or about October 10, 2023, the employer did not maintain powered industrial truck certification or records to ensure training and evaluations have been completed.  The GRM Information Management Services of Boston, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(6), which was contained in OSHA inspection number 1650251, citation number 1, item number 3 and was affirmed as a final order on August 8, 2023, with respect to a workplace located at 57 Littlefield Street Avon, MA 02322.
