29 CFR 1918.35: Employees were working around open weather deck hatches which were not protected by coamings to a height of 24 inches (0.61 m), or by taut lines or barricades at a height of 36 to 42 inches (0.91 m to 1.07 m) above the deck. M/V NYK Demeter, Berths 25-33, Port of Oakland: At Bay 50, aft, the catwalk adjacent to an open hatch was not equipped with adequate fall protection; the stanchions and lines did not meet the minimum height requirements above and were slack. The lines were measured with a tape to be 27" high. Employees are exposed to fall hazards.

29 CFR 1918.22(b): Each side of the gangway, and the turntable if used, did not have a handrail with a minimum height of 33 inches (measured perpendicularly from rail to walking surface at the stanchion) and a midrail. M/V NYK Demeter, Berths 25-33, Port of Oakland: The top rail is slack on the outboard side and less than 33" in height. Employees are exposed to slip, trip and fall hazards.

29 CFR 1918.22(c): The gangway was not kept properly trimmed M/V NYK Demeter, Berths 25-33, Port of Oakland: The space between the foot of the gangway and the apron had a measured vertical gap in excess of one foot and was not substantially parallel to the dock. Employees are exposed to slip, trip and fall hazards.
