M/V ONE Aquilla: On and/or before August 17, 2023, employee(s) utilized the gangway to access the container ship ONE Aquilla and conduct lashing operation. Employer did not ensure the netting was extended to cover the gap in between the dock and the ship which potentially exposed employee(s) to fall hazards.

Employer had the following: a) Catwalk Bay 18, forward end top tier: Lashing gear was not stored in the rod holders and were put in the center of the catwalk. b) Catwalk Bay 55, forward end top tier: Lashing gear was not stored in the rod holders and were put in the center of the catwalk. Employee(s) were potentially exposed to trip hazard(s).
