29 CFR  1910.36(a)(3):An opening into an exit was not protected by a self-closing fire door that remains closed or automatically closes in an emergency upon the sounding of a fire alarm or employee alarm system.   a) Back, right corner of the garage: On or about January 07, 2023, and times prior, the hardware on the fire door located at the back of the garage was disassembled resulting in the fire door not self-closing.

29 CFR  1910.157(c)(1):Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  a)  Back, right corner of the garage: On or about January 07, 2023, and times prior, portable fire extinguishers were not mounted and located so that they were readily accessible to trained employees expected to utilize fire extinguishers to fight small, incipient stage fires, exposing the employees to fire hazards  b) Front exit, near the office space.
