29 CFR  1926.300(b)(1):Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:  a) At 795 McFarland Parkway Alpharetta, Georgia, on or about February 7, 2023 where a worker was observed as exposed to amputation hazards from rotating unguarded starter recoil, in that the mortar mixer was not equipped with a pull start/recoil guard as a strap was used to start the equipment.

29 CFR  1926.452(c)(4): End frames of tubular welded frame scaffolding were not locked together vertically by use of pins or equivalent means in areas where uplift could occur:  a) At 795 McFarland Parkway, Alpharetta, Georgia, on or about February 7, 2023 workers performing masonry activities from the fabricated frame scaffold were exposed to approximately 13-feet to the concrete lower level in that the employer did not ensure the use of horizontal connecting pins where uplift could occur during loading and unloading of materials including but not limited to block and mortar pan by the powered industrial truck.
