On or about February 9, 2023, at a workplace located at 700 Industrial Street in Gresham, WI; the employer did not develop and document energy control procedures for when employees conducted servicing and maintenance of equipment with the potential for hazardous energy. Specifically, employees conducted preventative maintenance of the Weeke Machine Center CNC (Model Optimat BHP008/VANTECH512) which exposed employees to caught-between and struck-by hazards.
