29 CFR  1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section:   Note: Exception: The employer need not document the required procedure for a particular machine or equipment, when all of the following elements exist: (1) The machine or equipment has no potential for stored or residual energy or reaccumulation of stored energy after shut down which could endanger employees; (2) the machine or equipment has a single energy source which can be readily identified and isolated; (3) the isolation and locking out of that energy source will completely deenergize and deactivate the machine or equipment; (4) the machine or equipment is isolated from that energy source and locked out during servicing or maintenance; (5) a single lockout device will achieve a locker-out condition; (6) the lockout device is under the exclusive control of the authorized employee performing the servicing or maintenance; (7) the servicing or maintenance does not create hazards for other employees; and (8) the employer, in utilizing this exception, has had no accidents involving the unexpected activation or reenergization of the machine or equipment during servicing or maintenance.   On or about February 9, 2023, at a workplace located at 700 Industrial Street in Gresham, WI; the employer did not develop and document energy control procedures for when employees conducted servicing and maintenance of equipment with the potential for hazardous energy. Specifically, employees conducted preventative maintenance of the Weeke Machine Center CNC (Model Optimat BHP008/VANTECH512) which exposed employees to caught-between and struck-by hazards.

Employees capable of performing lockout and maintenance on equipment with hazardous energy such as, but not limited to electrical and pneumatic energies, were exposed to potential caught-between and struck-by hazards.

29 CFR  1910.212(a)(1):  One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:  On or about February 9, 2023, at a workplace located at 700 Industrial Street in Gresham, WI; the employer did not provide one or more method of machine guarding to protect employees from:  a)  Caught-between and struck-by hazards in the space between the discharge table, cutting table and control center for the Weeke Machine Center CNC (Model Optimat BHP008/VANTECH512). Employees were exposed to hazards posed by the moving carousel of the CNC machine and/or by the sheet products it pushed off its cutting table towards the discharge table.   b)  Entanglement, abrasion and laceration hazards posed by the rotating disk and spindle of the JET 20" disc sander (Model JDS-20-3) due to the sander's misaligned guard.

29 CFR  1910.334(a)(2)(i):  Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated:  On or about February 9, 2023, at a workplace located at 700 Industrial Street in Gresham, WI; flexible cord sets (extension cords) were not visually inspected before use for external defect. In the Fabrication area, extension cords with damage to their outer jacket were not inspected. Extension cords were exposed to damage through use and high-traffic areas.
