29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)   At a temporary jobsite located at 12604 W 160th St, Olathe, Kansas, employees performing stucco work were exposed to fall hazards while working from a fabricated frame scaffold. The employees were exposed to a fall distance of approximately 8 feet from the working level of the scaffold to the ground below in that working levels of scaffolds were not fully planked or decked.

29 CFR  1926.451(g)(4)(i):29 CFR 1926.451(g)(4)(i):  Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:  At a temporary jobsite located at 12604 W 160th St, Olathe, Kansas, employees performing stucco work were exposed to fall hazards while working from a fabricated frame scaffold. The employees were exposed to a fall distance of approximately 8 feet from the working level of the scaffold to the ground below in that fall protection was not utilized.
