29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  (a) G Mill - On or about February 27, 2023, the employer did not ensure that floors and elevated non-floor surfaces were maintained in a clean and orderly condition. Caseinate powder was allowed to accumulate on equipment, floors, and overhead pipes exposing employees to potential fire, deflagration, and explosion hazards.  (b) 2nd floor, Extruder Room - On or about February 27, 2023, the employer did not ensure that floors and elevated non-floor surfaces were maintained in a clean and orderly condition. Caseinate powder was allowed to accumulate on equipment, floors, and pipes, exposing employees to potential fire, deflagration, and explosion hazards.

29 CFR  1910.132(d)(1)(i):The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:   (a) 2nd Floor Extruder Room- On or about January 5, 2023, the employer failed to require employees to equip eye protection when handling Mississippi Lime Company Mississippi Lime Hydrated Lime, a powdered caustic chemical, resulting in an operator receiving a chemical burn to their left eye.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
