The employer did not locate or use in such a manner all energy isolating devices that are needed to control the energy to the machine or equipment to isolate the machine or equipment from the energy source(s).  a)  Lippman- Milwaukee 51x65 LS Crusher Plant. On or about February 28, 2023, and times prior, three employees used a Husky pry bar to hold the upper frame of the crusher plant open while performing maintenance. Employees were exposed to struck by, crushing injuries and death due to the gravity of the upper frame falling from not using the manufacturer provided safety locking pins.
