On or about March 1, 2023 at the address of 331 North Main St. Findlay OH, 45840: The employer did not ensure each employee working near walls where windows encasements were removed and the inside bottom edge of the window openings were 21 inches (1.0 m) above the walking/working surface, were protected from falls by the use of a guardrail system, a safety net system, or a personal fall arrest system. Employees performing framing work activities in proximity of the unguarded opening were exposed to fall hazards of between 16' - 30' to the ground below.
