29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift:   (a)   Off Honey Ridge Rd: On or about March 2, 2023, the employer exposed employees to a fall hazard in that a personal fall arrest or travel restraint system was not used by an employee working from a bucket elevated approximately 48 feet 8 inches above the ground.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a)    Off Honey Ridge Rd: On or about March 2, 2023, the employer exposed employees to a struck-by hazard in that eye protection was not used when while trimming tree limbs from inside a bucket truck.
