29 CFR  1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems.  a) Dish Pit; On or about March 7, 2023, and at times prior, the employer exposed employees to trip hazards, in that the floor drain cover had been removed creating an open floor hole while employees traveled in and out of the kitchen area.  b)Kitchen in front of sink; On or about March 7, 2023, and at times prior, the employer exposed employees to trip hazards, in that the floor drain cover had been removed creating an open floor hole while employees traveled in, out, and throughout the kitchen area. Employees use the panel to turn power on and off to the kitchen and exterior.
