At the jobsite, employees installing Tri Built synthetic underlayment to a new residential house was observed wearing fall protection but not tied off, exposing employees to an unprotected fall hazard at a height of over 6 feet to the ground below.  Blas Gutierrez was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1626315, Citation Number 1, Item number 2, and was affirmed as a final order on November 16, 2022, with a final abatement date on November 18, 2022,  with respect to a workplace located at 32156 Gillstar Haslet, TX 76052.
