29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about April 10, 2025, employees installing roofing shingles on a single-family house and using pneumatic nail guns without wearing safety glasses.  The Blas Gutierrez Mendez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number_1626315, citation number 1, item number 1  and was affirmed as a final order on 11/16/2022, with respect to a workplace located at 2156 Gillstar Haslet Tx 76502 .  The Blas Gutierrez Mendez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1770305, citation number 1, item number 1 and was affirmed as a final order on 3/13/2025, with respect to a workplace located at 940 Camelia Grove Ct Hutchins Tx 75141.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about April 10, 2025, employees doing roof work on a single-family house and accessing the roof by using an extension ladder that did not extend 3-feet above the landing.
