29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. On the roof of a temporary commercial worksite, located at 2550 Gilmore Street (Central Riverside Elementary School) Jacksonville, FL 32205: On or about April 7, 2023, the employer exposed its employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employee, while using pneumatic nail guns to install roofing shingles.

1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a. On a 6:12 pitched roof of a commercial worksite, located at Central Riverside Elementary School, 2550 Gilmore Street, in Jacksonville, Florida 32205: On or about April 7, 2023, the employer exposed employees to fall hazards ranging from 10 to 30-foot, in that employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards, while these employees were installing roofing materials such as, but not limited to shingles.
