29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  Worksite located at CCR Products, LLC, 175 South Street, West Hartford, Connecticut:  On, or about, April 1, 2023 and at time prior to, employees were exposed to a fall hazard while opening and closing a set of double doors that allowed pallets to be moved from the storage area to the ground floor of the building by a forklift.

29 CFR  1904.41(a)(1): The establishment had 250 or more employees at any time during the previous calendar year, and the establishment was classified in an industry listed in appendix A to subpart E of this part, and the employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee by the specified date:   Located at Triumph Engine Control Systems, LLC, One Charter Oak Boulevard, West Hartford, Connecticut:  On, or about, April 17, 2023, the employer failed to electronically submit information from their 2022 OSHA Form 300A or equivalent by March 2, 2023.  The establishment employed 530 employees during calendar year 2022.
