Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  (a) Worksite - right, front exterior of the house: On or about 4/27/2023, the pump jack scaffold was not erected by a trained and experienced employee under the supervision of a competent person qualified in scaffold erection in that, the scaffold was missing bottom bracing.

(a) Worksite: On or about 4/27/2023, an employee was exposed to fall hazards of approximately 24 feet above the lower level while working from a pump jack scaffold system without any means of fall protection.
