29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   a.) 952 S. Braddock Avenue, Pittsburgh, PA 15221:  On or about May 8, 2023,  two employees were observed performing masonry operations while standing and walking on un-secured planks from a fabricated frame scaffold up to 25 feet above ground level. The employees were not using any parts of a personal fall arrest system while squatting and bending within the edge of the scaffold.

29 CFR  1926.452(c)(2): Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally: The cross braces shall be of such length as will automatically square and align vertical members so that the erected scaffold is always plumb, level, and square. All brace connections shall be secured.   a) 952 S. Braddock Avenue, Pittsburgh, PA 15221: On or about May 8, 2023 - Two employees were performing masonry operations on an outrigger work platform on a fabricated frame scaffold that was not braced by any equivalent means. Scaffolding missing pins and cross bracing.

29 CFR  1926.452(c)(4): End frames of tubular welded frame scaffolding were not locked together vertically by use of pins or equivalent means:  a) 952 S. Braddock Avenue, Pittsburgh, PA 15221: On or about May 8, 2023 -  Fully erected tubular welded framed scaffolding were missing pins at the end frames. The four missing pins were approximately 7 feet up from the ground tubular welded framed scaffold.

29 CFR  1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or be tagged "Do Not Use" or similar language, and shall be withdrawn from service until repaired.    On or about May 8 2023, 952 S. Braddock Avenue, Pittsburgh, PA 15221: Employees were observed using defective equipment. The fixed ladder was not inspected prior to usage. The extension ladder used for egress had an  indent on the sixth rung. Condition was observed on May 8, 2023.
