On or about May 11th, 2023, employees were exposed to struck-by or tip-over hazards.  Two operators operated a Yale propane forklift prior to completing an evaluation of their performance.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1910.36(d)(1):Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge.  On or about May 11th, 2023, the following exit doors were welded shut or otherwise inoperable, exposing employees to potential fire hazards:    a) Warehouse, southwest loading dock door, equipped with illuminated "EXIT" sign - barricaded with steel brackets and non-removable lumber.  b) Warehouse, south building door, equipped with illuminated "EXIT" sign and inoperable panic bar / alarm - welded at bottom with steel bracket. c) Warehouse, east side door, equipped with illuminated "EXIT" sign. Inoperable door and locking mechanism.  d) Warehouse, north central building door, equipped with illuminated "EXIT" sign. Inoperable door and locking mechanism.  e) Office coordinator to warehouse, north service door - inoperable,  No form of exit demarcations.  f) Vent room door - inoperable, welded from the outside. No form of exit demarcations.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).  Gulfside Supply, Inc. dba Gulfeagle Supply was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.36(d)(1), which was contained in OSHA Inspection number 1635443, Citation 1, Item number 1a and was affirmed as a final order on April 18, 2023, with respect to a workplace located at 1820 N. Central Avenue, Chicago, Illinois 60639.
