29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about May 9, 2023 at the above addressed jobsite, the portable ladder used to access the roof extended approximately one foot above the upper landing surface, thereby exposing employees to the hazards associated with falls.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about May 9, 2023, at the above addressed jobsite, employees were performing roofing operations approximately 11.4 feet above ground and employees were not using fall protection, thereby exposing employees to fall hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).  AM Exteriors Pros, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1530757 , citation number 1, item number 1 and was affirmed as a final order on November 9, 2021, with respect to a workplace located at 7549 S. Creiger Avenue, Chicago, IL 60617.  AM Exteriors Pros, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1414083, citation number 1, item number 3 and was affirmed as a final order on August 6, 2019, with respect to a workplace located at 1327 Irving Avenue, Wheaton, IL 60187.
