29 CFR 1926.25(a):During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: Location: Around the outside of the Jobsite and on the stairwell platforms On or about May 17th, 2023 employees were exposed to slips, trips, falls, and puncture injuries while walking around the jobsite where lumber cut offs and 2x4s with protruding nails were not kept clear of work areas and walking pathways.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person. Location: In the Stairwell Opening On or about May 17th, 2023 employees were exposed to falls from heights up to 16 feet to the ground while accessing the 3rd floor where the temporary elevated platform erected in the stairwell opening was not built or inspected by a competent person qualified in scaffold erection. The platform was supported by 2x4s laid on the 1.5 inch side, planked with unsecured, overlapping plywood, and a 1 foot gap was present between the hallway and the platform.
