29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily: Powered industrial trucks used to move materials located at 2345 U Road, Sabetha, Kansas were not inspected daily to discover equipment defects prior start of shift. A caterpillar forklift in the West shop production department had a defective horn, thus exposing workers to struck-by hazards.

The unused portion of the bandsaw blade was not guarded.
