29 CFR 1926.501(b)(10): Employees engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail systems, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system:   (a)L & L Exteriors, LLC at 3005 Beale Street, Rapid City, SD 57703: On or about and at times prior to May 23, 2023, the exposing employer did not ensure that employees were protected from falls while conducting commercial roofing activities on a commercial building. This condition exposed the employees to fall hazards greater than six feet.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  (a)L & L Exteriors, LLC at 3005 Beale Street, Rapid City, SD 57703: On or about and at times prior to May 23, 2023, the employer did not ensure a written training certification on the use of fall protection systems prior to employees being exposed to fall hazards while working on a commercial property. This condition exposed employees to fall hazards greater than six feet.
