29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  At the construction site located at 808 Laclede St, Lebanon, Missouri employees were exposed to fall hazards.     a) At the construction site located at 808 Laclede St, Lebanon, Missouri employees were exposed to a fall hazard.  A drywall finisher unloading materials from deck on the southeast  corner of the building was working at height.  The deck had no guardrails in place to keep the employee from falling to the ground, which was 21 feet below. No alternative means of fall protection was in use.    b) At the construction site located at 808 Laclede St, Lebanon, Missouri employees were exposed to a fall hazard.  A drywall finisher unloading materials from job made trash box was working at height.  The employee was not wearing a personal fall arrest system while standing in the box, which was approximately 21 feet above the ground. No alternative means of fall protection was in use. This resulted in fatal injuries when the box tipped off the forks.
